1. Identify what brought your patient to the hospital (main concern) and their PHMX.
2. Identify the current priority medical/surgical diagnosis. This may be different from the original admitting diagnosis.
I-Name and define the current, priority medical/surgical diagnosis
II-From the clinical manifestations of this diagnosis, identify the signs and symptoms of the diagnosis that the patient is exhibiting. Briefly explain them in pathological terms.
III-List the diagnostic test results relevant to this diagnosis. Include both lab test results (e.g. bloodwork) and other diagnostic tests (e.g. xrays). Briefly explain how each test result relates to the pathophysiology of the diagnosis for this patient.
IV-For your Patient, identify the medical treatments currently ordered by the doctor. Include treatments to cure or control the diagnosis and the patients PMHX, manifestations and potential complications

b)Other interventions/treatments
c)Assessments specific to monitoring this diagnosis, its manifestations or potential complications.
3. List all relevant physical and psychosocial (10 total) Nursing Diagnoses(NDx) based upon both the analysis of the priority medical/surgical diagnoses and the Head to Toe patient assessment data. Include both actual and potential ‘at risk for’ NDx.
I-Based on the list above identify the priority NDx (one physical and one psychosocial) including the etiology (‘related to’) and the manifestations (‘as evidence by’)
II-Provide a rationale for both selections.
III-Identify both the short-term and long-term expected outcomes for each of the two priority NDx. Write these expected outcomes using the SMART criteria.
IV-Identify realistic and therapeutic nursing interventions (using the subtitles Assessment, Pharmacological, Independent, Collaborative, Health-Teaching) ) for each of the priority Nanada diagnoses in relation to the expected outcomes identified.
V-Identify evidence-based rationales for each listed intervention.
Assignment submissions must follow APA guidelines. A minimum of five references must be used including one peer-reviewed journal (less than five years old).
