I have attached a “sample case log” as to what it is supposed to look like. Please only use it as a reference. It is another student’s work from the past and will come up as PLAGIARISM if you use it. DO USE THE HOSPITAL PAPERWORK that I have attached to this to be able to fill out the case log for the patient. Patient’s initials are V.V. As long as all of the patient”s information is typed up from the hospital paperwork and you use the “sample case log” as a reference, I will look over it and give you a BONUS $$$. It must be at least 4 full double spaced pages and submitted by 1245pm in order to receive the bonus. If you have to make up things or add things to it that are not there, that is fine.
