Introduction:
I was allocated to collect a patient for an open tibial fracture surgical procedure. When arrived at the ward I went through the patient’s medical folder in order to check his consent form, pre-op checklist and all relevant information needed prior to taking the patient to the theatre. The checklist was appropriately filled and signed by the nursing staff and the consent form was signed by both the responsible clinician and the patient.
I approached the patient, introduced myself and asked him how he feels. After receiving an answer I explained that I was a staff member of the theatre team that will be looking after him. I asked him if he would answer a few questions before going to the theatre and he said that he would and can not wait for everything to be sorted out.
While going through the questions from the theatre checklist I noticed that there was wrong information recorded about the patient being Nil By Mouth from 2 am. The form stated that the patient had nothing to eat from 2 am, but when I asked him when was the last time he had something to eat he said that he had two bites of a sandwich given to him by a staff member. Therefore my duty was to inform the patient that I need to let the theatre and anaesthetic team know about that and I would get back to him.
