I have attached a “sample case log” as to what it is supposed to look like. Please only use it as a reference. It is another student’s work from the past and will come up as PLAGIARISM if you use it. USE THE HOSPITAL PAPERWORK that I have attached to this to be able to fill out the case log for the patient. It must be at least 4 full double spaced pages. If you have to make up things or add things to it that are not there, that is fine. THANKS!
