Implicit biases are unintentional automatic brain processes that cause associations and responses; agents are unaware of the associations with a stimulus. Numerous researchers concur that social status is relational and that individuals subconsciously have more negative attitudes or feelings toward belonging to an outgroup than toward belonging to an ingroup. Beyond stereotypes, implicit bias includes positive or negative assessments of various groups of individuals. The unequal treatment or effect of general policies, practices, and norms on people and communities based on social group membership results from either implicit or explicit biases. Healthcare and public health professionals responsible for population health and wellness continue to have a fundamental professional obligation to address bias (Vela et al., 2022).
There are several things a provider can do to provide equitable care and avoid biases when caring for patients with chronic diseases. One is for providers to be aware of their feelings and biases when caring for patients with chronic illnesses. Active listening and accepting each patient individually and using Jean Watson Caring Caritas can make a difference in how the provider views each patient as an individual (Watson, 2022). Providing a therapeutic caring environment can create a positive experience for the patient and provider. For chronic diseases where evidence-based clinical practice guidelines exist, tailoring these recommendations to patients’ preferences is a starting point to ensure that patients receive equitable and the best-recommended care. When using clinical practice guidelines, caution must be taken, as some may inadvertently disadvantage some populations. Providers must know this, especially when guidelines and recommendations are beyond patients’ resources (Eslava-Schmalbach et al., 2017).
A primary care system that is already under pressure from an aging population and an increase in the prevalence of chronic diseases is made even more taxing by the burden of managing complicated patients with chronic conditions in community settings. Due to a lack of primary care physicians and the rapid growth in the supply of nurse practitioners (NPs) and physician assistants (PAs), NPs and PAs are playing a more significant part in the treatment of these patients. Nurse practitioners can promote equitable care by integrating some core values, like practicing with a sense of regard and decency. Remember that all patients are valued, and their efforts are encouraged (Morgan et al., 2019).
