Once you have completed the readings above, re-read the Case Scenario from the Talebreza & Widera (2015) article and discuss the following questions with your peers:
If you were the spouse of this patient, how would you move forward in making a medical decision?
What important questions would you need to ask?
Would palliative care have a place in this situation? If so, what would be the benefits?
What would be your outcome in this situation?

If I were the spouse of this patient, I would consult the physicians at the hospital and may ask for a second or third opinion from an outside consultant as well. Some of the questions would be the following:
What can be expected to happen in the next few hours, days, or weeks if current treatment continues? What can be expected if it is withdrawn?
What would be his projected lifespan in either scenario?
What is the likelihood he could survive and not require long term skilled nursing care?
Will he know who his family members are?
What activity of daily living will my husband be able to perform?
When would he become eligible for hospice and how would that impact his care?
What option will provide the least suffering for my husband?
A palliative care team would be helpful â especially if engaged prior to this scenario. Presumably a palliative care team would establish a staged plan of care that could outline disease progression and applicable interventions. It could also review any advanced care directives and see if further detailed scenarios should be outlined regarding specific prognosis. The family would benefit from palliative care services that consider the needs of all individuals involved â a patient and a family centered care plan could ensure responsibilities of not just providers but also the patient and their caregivers are clearly understood. Family education improves QoL as it effectively increases the independence and spirituality of caregivers in regard to abilities to meet the needs of the patient, implement effective coping strategies, understand advance care planning, as well as end of life preparation (Al-agamy et al., 2022). If palliative care had been implemented earlier, the wife would likely not be troubled by this dilemma but rather know definitively which actions are reflective of her husbandâs wishes. This would provide practical comfort and allow her to focus on the emotional aspect of witnessing the suffering of a loved one rather than have that compounded or clouded by the stress of her uncertainty of whether she is âmaking the right decision.â Ultimately, I would assess the answers provided by the healthcare professionals to determine whether to proceed or not. I would factor in hopefully knowledge gained over the years of living with my husband whether or not he would want to âfight onâ or âavoid further suffrage.â The answer to my last question would weigh more heavily than others and likely determine my decision.
