Below are specific scenarios when I used my clinical nursing judgement. Please elaborate on these topics and compare/correlate them to the importance of having clinical nursing judgement.
One scenario that stuck with me since the beginning of nursing school was during my first clinical rotation on a medsurg unit. My clinical instructor had asked me to interview my patient about his finances and how he was going to pay for his stay at the hospital. While I understand these are viable questions as part of a psychosocial assessment, my patient was a terminally ill cancer patient. He was suffering from end-stage colon cancer and was going to be transferred a hospice location to receive end of life care. Given my patients state of well-being, he was not going to be able to answer my questions. I was going to have to discuss these topics with his wife instead. Using my clinical nursing judgement, I decided to stand up to my instructor and not conduct this interview as I felt it was inappropriate and morally wrong. His significant other was already visibly upset, and I did not see this an appropriate conversation to engage in. This was not easy for me to discuss with my instructor as it was my first time working as a nursing student in the hospital setting. It was an intimidating situation. In hindsight, I am proud of myself for using my better judgement and not following through with what my clinical instructor insisted I do.
Another scenario that sticks out to me happened last semester during my critical care rotation in the SICU. My patient was ordered digoxin to treat his diagnosis of atrial fibrillation. Upon my initial assessment, I noticed his heart rate was trending in the low 60’s. When it was time to pass meds to my patient, I once again assessed his heart rate. My assessment revealed his heart rate was 56 beats per minute. Using my clinical nursing judgement, I decided not to administer him the digoxin and hold medication. I then contacted his physician and documented in the EMR that he did not meet the parameters for administration of this medication since his heart rate was less than 60 beats per minute.
