Direct Data (Observation: Based on Sight, Sense, Smell, Sound): A windshield survey
gives public health nurses a quick assessment of the community or area in which they
practice. Through the windshield survey, nurses obtain qualitative data about the community’s
socio-economic, environmental, and cultural vitality and needs by using their five senses.
Findings from your windshield survey are considered direct data.
The first section of your paper should address each of the categories of the Windshield
Survey Assessment (housing and zoning; open space; boundaries; commons;
transportation; service centers; stores; street people; signs of decay; race; ethnicity; religion;
health and morbidity; politics; media).
Indirect Data: (Based on Data Sources)
Indirect data collection includes gathering statistics for your community (demographic,
socioeconomic, environmental, epidemiological, and cultural data), should mirror the categories
in your windshield survey, and be current and referenced. From both the direct and indirect
data risk factors for the community should be identified and critically evaluated.
o Report on collected population statistics for your selected community, county, state,
and national level.
o Locate and report on health risk factor data for your selected community as well as
pertinent corresponding data at the county, state, and national levels.
o This data you discuss should be current as possible and referenced.
Data Interpretation/Analysis
o Compare and contrast population health and risk factor data at the local, state and
federal level.
o Identify, explore and discuss community resources (availability, accessibility, purpose)

o Identify the strengths and needs of the community.
o Identify three (3) deficits/needs (Nursing Diagnoses which will be in community
health format) based on your data collection, interpretation and analysis.

Community/Aggregate Plan of Action: Select a primary need for your community
(nursing diagnosis) and develop a brief care plan to address this need.
