I chose improve patient documentation (can be achieved through electronic health records/ bedside shift report/ required documentation list/ proper orientation) implement a discharge planning process (can be achievedd by consulting case mangement/ proper assessments to determine pt readiness) and decrese the incidence of ventilator associated pneumonia (which can be achieved by shortening time of ventilator- wean pt and perform spontaneous breathing trials daily/ daily ABGs/ daily chest x rays/ DVT prohylaxis/ stress ulcer prophylaxis/ nutrition consult, decrease risk of infection- perform frequent oral care/ elevate HOB to at least 30 degrees/ turn q2.
