Using the case study provided (in attachment Valery file) and with reference to an evidence-base throughout
– Discuss the role and value of the care planning process.
– Identify a nursing model for use in care planning and explain your choice for the person in your case study.
– In order of priority, plan three evidence-based care goals, with a minimum of four interventions per goal, and state when the interventions need to be evaluated. Refer to SMART theory to help formulate the care plan. Discuss the rationale for prioritising each goal.
– Incorporate a minimum of three psychosocial and / or biopsychosocial elements to support the goal discussion and reflect partnership working throughout.
– One intervention must have a multi-disciplinary team focus, one must have a nurse focus and one must have a patient/family focus.
– Need to use minimum 20 provided references rest

– 3000-word limit for essay!

 Why do we plan care, post our assessment of a patient/client?
 What are the benefits of a care plan?
 Why is it important to assess and plan in partnership with the patient/client, the family and the multi-disciplinary team?
 Why is it important for nurses to practice in an evidence-based manner?
You need to meet 3 learning outcomes.
LO1 Apply a person-centred approach to planning care working with the person in a multi-disciplinary team.
LO2 Formulate an effective and sustainable care plan based on the person’s assessed needs according to priority.
LO3 Search and apply the relevant evidence to support the development of an evidence-based care plan.
The care planning process
 Use a care-plan model as a guide.
 Explain three needs that you want to discuss and state a goal for each.
 For each of the three goals, state a minimum of four interventions.
 Use SMART theory to help outline your goals and interventions.
 Use an evidence-base to support all your decisions/ discussions.
 Explain why the interventions chosen will assist in meeting that goal.
 Support your reasons for the evaluation date planned.
 Why is a person-centred approach taken?
 Identify the relevance of the psychosocial or bio-psychosocial theory that you discuss and why nurses should be knowledgeable about such.
