Section 1: Introduction to Your community (10%)
In this section, you will share highlights of your community. What do you enjoy about your community? Items you may what to include but are not limited to include you community’s demographics (ratio of male/female, race, education, religion, etc.).
Section 2: Community Health (10%)
In this section, please share what constitutes a healthy community and how you measure signs of good and poor health within a community.
Section 3: Windshield Survey (15%)
The elements that are required in this section include:
What are the community characteristics
What are the health resources that are available in your community
What recreation, worship, food stores/farmers market and dining can be found in your community
What are the health concerns in your community
Section 4: Environment (10%)
There are many environmental hazards found in the home, work, and community. What type of environmental hazards did you discover in your community and what influence do these hazards have on the population?
Section 5: Emergency Preparedness (10%)
What types of disasters would specifically impact/threaten the community you live in? As an example when I lived in Florida the top 3 disasters that threatened my community included, Hurricane, Tornado, Flooding?
When natural disasters occur in your community, who in the community is responsible for making sure that everyone is aware of the response that needs to be taken by all parties? How is your community notified about the emergency? How often should and does your community practice drills be conducted?
Section 6: Personal Contribution (10%)
Please share how you are making a positive difference/impact, improving the health and wellness of your community.
If you had a magic Public Health wand, what one change would you make in your community to improve the lives of the members?
Section 7: Conclusion (10%)
Please provide a summary of your PPP, also include what new information are you taking away from our course. Has this information change your personal and professional prospective, in what ways?
