Assignment Description: Individually, students will develop an evidence-based care plan based on their assessment of an adult in their life. There are two parts to this assignment:
Part A: Individually, students will conduct an interview and obtain Assessment data (objective and subjective) then cluster the data to provide two Nursing diagnoses’.
Assignment Purpose: To demonstrate the use of the nursing process to organize data, plan client-centred care, and evaluate actions to develop an understanding of the LPN role.
Assignment Instructions:
Please review the attached marking guide and rubric for Part A (It would also be beneficial to look at the rubric for part B to see where the assignment is going).
Review Chapters 13 and 14 in Potter and Perry and notes are taken from class about the nursing process and care plans
Select one adult in your life (must be over 18 and consent to the project), and conduct a nursing interview; include subjective and objective data. The data you will obtain come from questions that are generated in class, from your textbook, or other scholarly articles. It would be ideal to conduct this interview face to face. However, if COVID and public health restrictions are in place, FaceTime or other video chat methods would be acceptable.
When your interview is complete, add your assessment findings to the assessment column on the nursing care plan for your client using the attached “Blank Nursing Process Template”. Then cluster your data and begin to think about 2 possible nursing diagnoses.
Considering your assessment data, develop two nursing diagnoses.
The care plan needs to include: relevant objective and subjective data and two nursing diagnosis
The assessment can be written in point form. The diagnosis needs to be written as a NANDA statement sentence.
Submit your nursing care plan (assessment and diagnosis) as one document into Blackboard assignment dropbox.
