Description of incident:
A missing swab at obstetric (18 x 18) left in the patient as a near miss. It was noticed during the first count. Patient was booked for an elective Lower Segment Caesarean Section who also had fibroids. Staffs got to know during the team brief and the necessary equipment was provided based on the EBL.
I was double scrubbing with my mentor and so I saw the swab left in the patient to soak her bleeding.
During the first count my mentor noticed of a missing swab. I told her itâs in the patient but the surgeon denied it yet I could see a bit of the string in the swab showing.
The surgeon kept denying and even shouted at me because I was insisting that the swab was inside the patient. She finally found it when she was about to close the sheet and apologised.
Incident should be attached as an appendix.
