Patient Medication Pass – Clinical Reflection Journal Questions
ALL students will utilize this set of questions for the clinical reflection journal assignment. The questions should be answered based upon your experience from your patient medication pass.
Section 1: Individualizing Patient Care
How could you go about supporting and encouraging patient preferences during medication administration to make care more individualized to each patient/family?
Section 2: Addressing Potential or Real Medication Errors
1. Discuss steps you would take if you suspect or recognize an error is occurring or has occurred in the administration of a drug to a patient.
2. Discuss measures you would take if you found an inappropriate medication order for a patient. What other members of the health care team would you involve in finding a solution to this problem?
Section 3: Locating Evidence to Support Practice
Describe what you would do if you had an order to give medication by a route of administration you had never had the opportunity to do. Where would you go to find research-supported information to guide you in giving the medication?
Section 4: Takeaways
Explain the most important thing you learned from this medication administration and how you will use this in caring for your patient?
1. Journals must be typed and be in APA format. They should be at least 1 ½ pages in length (not including the title page), double-spaced with margins at 1”, be in Times New Roman font size 12, and include a title page, title, page number, and headers.
2. EDIT YOUR PAPERS BEFORE SUBMITTING. Papers that are too short to adequately cover the subject or have misspellings or grammatical errors will be turned back for correction and receive a zero. Please use spelling and grammar checks.
3. Make sure you cover each section without repeating content in another section. The material should be thoroughly covered in a clear, brief, concise manner.
4. Remember, this is not a diary of everything you do that day in clinical. It is an evaluation of one moment that made you stop, think, and reflect on nursing practice.
5. At least one reference from a peer-reviewed journal (no older than 5 years) is a requirement for this journal.
Clinical Reflection Rubric
1. Topic Development— 50 pts Excellent – The topic is clearly stated and discusses all aspects of the assignment. The introduction, discussion, and summary are cohesive and substantive
2. Supporting Evidence – 30 pts Excellent–The writing shows logical progression and includes clear examples of key topics. Critical thinking and reasoning are demonstrated. Supporting evidence is nationally accepted guidelines, standards of practice or peer reviewed journals.
3. APA Formatting –20 pts Excellent -The writing is generally free from grammatical and APA errors. The journal and reference page formatting follows APA rules (title page, headings, etc).
