address is “Prevention of Malnutrition/Nutrient Deficiencies”. I did my observation hours at a nursing home and a
memory care assisted living facility. I found that there was a big concern for these patients with dementia not
getting the adequate amount of oral intake. This was caused by several reasons including, staff not encouraging
and assisting patients with meals everytime. Patient’s still wanting to be independent but not able to recognize how to pick up the food with the fork or spoon. Patients taking medications making them lethargic causing them to not
have the energy to eat. At these facilities they implemented monthly weights on all patients so they can track to
see if a patient is gaining, losing or maintaining their weight. Section “A” in the rubric is complete. I’ve attached the
rubric and everything must meet as “competent” or it will not pass.
