Pharmacy students are often faced with drug information questions from friends, relatives, colleagues, or patients at
work. In this assignment, we ask that you complete the Clinically Answerable Question
Intake/Tracking Form (See attached) to work up one of those clinical questions. The assignment can be completed using a question from any person you would
like but it has to come from a direct interaction with them (i.e. a question that comes up from a face‐to‐face interaction,
a phone call, or videoconference). The question can be any question relevant to drug therapy (prescription or self‐
medication) or supplement asked by a person.
This direct interaction between you and the person asking the question allows you to gather all the information that
goes into each section of the template and will help you ask follow up questions to locate the evidence in a more
efficient manner. Note that should the question initially asked by the person may not clear/complete and you may need
to probe further to clarify the question. You are not required to submit this template to the person asking you the
question (…although it would be nice to give them an answer on what you find nonetheless).
Formatting requirements:
1. Must use the provided template.
2. Maximum 2 pages single‐spaced, font size 11‐12, font type Arial or Calibri.
3. Submit as a Word document.
4. Make sure to include your name at the end of the document.
References: at least one have to be from primary literature and two additional references must be on MLA.
note: I already have the question and the template that is needed to complete the assignment, I only need help with the research and cheking the whole work.
